Stop remembering your questions in the parking lot.

What it looks like
From “what should I ask?” to a list you can use
You say what the visit is about. The answer comes from your own records: what the results show, what’s missing, and questions to ask.
Here’s what your records show.
- A1c went from 5.5% in 2023 to 5.8% in 2024 and 6.1% in May. The lab’s upper limit is 5.6%.
- Your 2022 intake notes mention that your father has type 2 diabetes.
- Your note from last year’s physical records afternoon tiredness.
- I don’t see a lipid panel in your records from the last two years.
Questions you could bring:
- My A1c has risen three years running. At what point would you act on it?
- Given my father’s history, is there anything else you’d check?
- Could this be connected to the tiredness I mentioned last year?
Worth bringing: the three A1c results and your medicine list.
Why it’s hard
Appointments go fast
The question you meant to ask comes back to you in the parking lot.
A new doctor has a short visit to catch up on years of your history. Your results are split across portals, and the one from two years ago lives in a system you can’t log into anymore.
So you try to remember the numbers, summarize your history on the spot, and fit in the thing you came to ask about. Notes would help. Making them is the hard part.

How it works
Set it up once, ready for every visit after
-
Start your trial
Say who the profile is for.
-
Bring your records in
Ask Triangle to request them through the national networks (you verify it’s you, sign one release, and meet a clinician who signs the request with you), or upload what you already have, like recent results or visit notes. Or both.
-
Write your questions
Tell the chat what the visit is for. It helps you put your questions in writing, with the results worth bringing.
What you get

- Your history, summarizedA plain-language summary of your medical history, short enough to read in the waiting room.

Questions
Good to know
- What does it cost?
- A 7-day free trial, then $9 a month. You add a card to begin; cancel before day 7 and you’re not charged. After that, cancel any month; your records stay yours. Membership covers Triangle requesting your records and keeping them updated, the chat that answers from your own records, a licensed clinician’s review of your history, wearables and uploads.
- My appointment is soon. Will it be ready in time?
- Start with what you have. Upload recent results or visit notes during setup and ask about them. Records requested through the networks arrive as each provider responds, and Triangle can’t promise when, so bring what you already have to this visit. Your history keeps filling in for the next one.
- Will it reach all my doctors?
- Triangle requests records through the national networks, Carequality and TEFCA. Not every provider can be reached that way. Firstly shows what may be missing and helps you request it, and you can upload the rest.
- Will Firstly tell me what’s wrong, or what to take?
- No. Triangle does not diagnose, treat or prescribe, and it never replaces your care team. Firstly helps you understand your records and prepare for the conversation; your doctor makes the decisions with you.
